Please complete and send this form before your first appointment. If you are unable to attend your appointment for any reason please contact me asap to rearrange or cancel. Cancellations within a 24 hour period of the session time will be charged at half the session cost. Cancellations prior to 24 hours notice will incur no cancellation fee.

Before your first hypnotherapy session, I offer a short 15 minute telephone call. This gives us an opportunity to discuss what you would like help with, answer any questions you may have and make sure hypnotherapy is appropriate for you.

Please contact me before your appointment if you become unwell, develop an infection or contagious illness, or experience any significant change in health after completing this form.

If there is anything you are unsure about or feel may affect your hypnotherapy session, please contact me before attending.
